Edison 2023-24 Registration Forms
Returning Students: Please fill out forms 1-6, and forms 10-13 if applicable.
New Students: Please fill out forms 1-9, and forms 10-13 if applicable. New students must also submit a copy of the student's birth certificate, and 2 proofs of home address (2 proofs of address can be a valid state ID/drivers license with your current address, as well as a utility bill or a voter registration card).
