Returning Students: Please fill out forms 1-7, and forms *11-14 if applicable.
 
New Students from a previous CPS school: Please fill out forms 1-8, and forms *11-14 if applicable. New students must also submit a copy of the student's birth certificate, and 2 proofs of home address (2 proofs of address can be a valid state ID/drivers license with your current address, as well as a utility bill or a voter registration card).
 
New Students from a non-CPS school: Please fill out forms 1-10, and forms *11-14 if applicable. New students must also submit a copy of the student's birth certificate, and 2 proofs of home address (2 proofs of address can be a valid state ID/drivers license with your current address, as well as a utility bill or a voter registration card).
 
*Form 13 is the CPS Student Transportation Form. For the 2023-24 school year, Edison did not have CPS bus transportation. We do not anticipate having CPS transportation for the 2024-25 school year. However, we are sharing this application so that we can still assign you for transportation in case the bus situation is resolved.
 
 
 
New Students: Please submit your registration forms by July 1st to the Edison office. If it is after July 1st, please contact the Edison office to schedule an appointment to drop off your forms.
 
Returning Students: Please submit your registration forms on Registration Day (Date: TBD, mid-August)
