Edison 2025-26 Registration Forms

 
Returning Students: Please fill out forms 1-7, and forms *11-14 if applicable.
 
New Students from a previous CPS school: Please fill out forms 1-8, and forms *11-14 if applicable. Also, please let your current CPS school know that you have accepted an offer with Edison. New students must also submit a copy of the student's birth certificate, and 2 proofs of home address (2 proofs of address can be a valid state ID/drivers license with your current address, as well as a utility bill or a voter registration card).
 
New Students from a non-CPS school: Please fill out forms 1-10, and forms *11-14 if applicable. New students must also submit a copy of the student's birth certificate, and 2 proofs of home address (2 proofs of address can be a valid state ID/drivers license with your current address, as well as a utility bill or a voter registration card).
 
*Form 14 - Transportation Service Request Form: Please fill out this form if you are interested in student transportation in the case Edison receives bus service for the upcoming school year. *Edison has not received bus service since the 2022-23 school year, and we do not anticipate any changes.
 
 
 
New Students: Please submit your registration forms by June 16th to the Edison office. If it is after June 16th, please contact the Edison office to schedule an appointment to drop off your forms.
 
Returning Students: Please submit your registration forms on Registration Day (Date: TBD, early-August)
